I am an ARRS Care Coordinator, supporting frail and elderly patients at Bersted Green Surgery, within local care homes, and across the community. My role helps to bridge the gap between social care and the GP surgery, ensuring patients receive the right care, support, and guidance at the right time.

I also assist with ReSPECT conversations and future care planning, helping patients and their families make informed decisions about their health and wellbeing.
